Have you had contact with anyone with confirmed COVID-19 in the last 14 days?
Have you had any of these symptoms in the last 14 days?
- Fever greater than 100
- Difficulty breathing
- Cough
Are you currently experiencing fever over 100, difficulty breathing or cough?

If you answered yes to question 3 or 4 please do not come into the office until the symptoms have subsided.

At this time, in order to abide to the new rules and regulations, we need to have scheduled appointments only. We can try to facilitate walk ins but if we are at capacity we may ask that you wait in your car or schedule for another time. Please call the office to make your appointments.
